1 | Legs | Your legs fuse together into one limb, giving you disadvantage on Dexterity saving throws and halving your movement speed. In addition, whenever you move more than 5 feet, you must succeed on a DC 15 Dexterity saving throw or fall prone. |
2 | Mouth | Your mouth sprouts cruel-looking fangs and you are overwhelmed by a powerful wave of hunger for blood. Whenever you make an attack, you must succeed on a DC 20 Constitution saving throw or instead target the closest creature to you with the following attack using your attack bonus: Frenzied Bite. Melee Weapon Attack: reach 5 ft., one target. Hit: 18 (4d8) + your Strength modifier piercing damage. If no creature is in range, you must use your movement to move as far as possible toward the closest creature, wasting your action if you still are not close enough. |
3 | Hair | Your hair begins to grow at a frighteningly rapid rate, giving you disadvantage on Dexterity-based ability checks, Dexterity saving throws, attack rolls, and Perception checks based on sight. In addition, the longer it grows, the slower you get. For every minute that passes, your hair grows 5 feet longer, and your movement speeds are reduced by 5 feet, with a minimum of 0. |
4 | Eyes | Your eyes deform and nauseating tumors protrude from the sockets, distorting your brain waves. You instantly become blind and have disadvantage on Intelligence saving throws. In addition you become unable to benefit from blindsight in any way. |
5 | Arms | You instantly grow a demonic-looking, sentient third arm. Its alignment is chaotic evil and it hates you with a burning passion. The arm shares your ability scores and proficiencies, it has a proficiency bonus of +5, and it can communicate with you telepathically in a horrible, grating voice. It acts immediately after you on your turn, and can access any of your items or weapons that you can access and are not currently holding. In addition, whenever you make an attack or use an item, the arm can force you to make a contested Strength check against it, causing the attack or item use to fail if you lose. |
6 | Stomach | Your stomach begins to boil with increasingly intense, burning acid. At the start of each of your turns, you take 11 (2d10) acid damage. In addition, whenever you move more than 15 feet or make a Dexterity saving throw, you must succeed on a DC 20 Constitution saving throw or instantly spew a Black Dragon's acid breath weapon in front of you. |
7 | Blood | Several of your blood cells are mutated into spider-like organisms that crave only the destruction of your body from the inside out. Any time dice are rolled to heal you, you take damage equal to the number rolled instead. In addition, whenever you make a Constitution saving throw, you automatically fail and take damage equal to the number you rolled on the d20. |
8 | Brain | Your brain dissolves into little more than mush, leaving it ripe for molding. Your intelligence score is reduced to 0 and you are forced to waste your turns doing nothing unless you are told by a creature to take some other course of action. You are forced to blindly comply with the request, but can only be commanded by a creature in this way once on its turn, and only on its turn. |
